Pharmacokinetics
The loop agents are rapidly absorbed and handled by renal excretion and secretion. Duration of effect is for 3 to 4 hrs. The diuretic inhibits primarily the absorption of Sodium and Chloride in both the proximal and distal tubules and in the loop of Henle. The onset of diuresis is within 1 hour. The peak effect occurs within the first and second hour. The duration of action is 6-8 hours. The duration of diuretic effect is approximately 2 hours.

& Dosage
Oral
MILD TO MODERATE HYPERTENSION: 40-80mg once or twice daily.
CARDIAC, PULMONARY, RENAL HEPATIC AND CEREBRAL OEDEMA: 20-80mg as a single
dose daily or on alternate days. Children: 1-2mg/kg/day as single dose.
IM/IV
CARDIAC, PULMONARY, RENAL HEPATIC AND CEREBRAL OEDEMA:
20-80mg/day IM or slow IV (1-2 min).
Children: 1mg/kg/day IM OR IV. Max daily dose 20mg
Action
Frusemide inhibits reabsorption
of sodium and chloride mainly in the medullary portionof hte ascending
limb of the Loop o Henle. Excretion of K+ and ammonia is also increased
while uric acid excretion is reduced. It increases plasma renin levels and
secondary hyperaldosteronism may result. Frusemide reduces BP in
hypertensives as well as in normotensives. It reduces pulmonary oedema
before diuresis has set in.
Interaction
It may increase the nephrotoxicity of cephalosporin and aminoglycosides. Phenytoin and indomethacin decreases the effectiveness of frusemide. Analgesics may reduce the natriuretic effect. Frusemide enhances muscle relaxants.
Adverse Effect &
Precaution
Fluid and Electrolyte
imbalance, nausea, anotrexia, vomiting, epigastric distress, fatigue,
weakness, dizziness, fatigue, weakness, muscle cramps, drowsiness,
dizziness, paraesthesia, hyperuricaemia, decrease in tolerance to
carbohydrates and deafness (transient). Hearing impairment and deafness
are usually reversible.
Allergies : Allergic reaction
to frusemide may occur. Pregnancy: In general, diuretics are not useful
for normal edema of feet and hands that occurs during pregnancy. Diuretics
should not be taken during pregnancy. Breast-feeding : Frusemide passes
into breast milk. Older adults: Dizziness, light-headedness, or signs of
hypokalemia may be more likely to occur in the elderly, who are more
sensitive to the effects of this drug. Elderly patients may also be more
likely to develop thrombii.

Man: Use with Caution.
