Pharmacokinetics
Ibuprofen
is phenylpropionic acid derivative. When given orally it is well
absorbed and is highly plasma protein bound. It gets metabolised in the
liver. It has a half-life of 2 hrs. About 10% gets excreted in the
urine.
Oral
MUSCULO-SKELETAL PAINS OF ALL TYPES INCLUDING TRAUMA, RA, OA AND
ANKYLOSING SPONDYLITIS, CHRONIC JUVENILE ARTHRITIS, ACUTE GOUT,
DYSMENORRHOEA AND OTHER PAINS: 1.2-2.4G/day in 3-4 divided doses. Chronic
juvenile arthritis: 30mg/kg/day in divided doses. Sust. Rel preparations
are usually give twice daily.
CHRONIC JUVENILE ARTHRITIS: 20-40mg/kg in div doses.
ANTIPYRETIC IN CHILDREN:(1-12yrs) 7-20MG/KG/DAY IN 3 divided doses.
Under 1yr: Not recommended
Anti-inflammatory, analgesic and antipyretic. Analgesic effect
is independent of anti-inflammatory activity and has both central and
peripheral effect. Temperature is reduced in febrile patients. Potent
inhibitor of cyclo-oxygenase and prostaglandin synthesis. Prevents
formation of thromboxane A2 by platetels. May delay onset of labour
Ibuprofen when used with Asprin gets displaced from its site . When used with lithium and Methotrexate, it increases their toxicity. Ibuprofen antagonises the effect of diuretics like Frusemide and Thiazide diuretics.
Adverse
Effect & Precautions
Nausea, vomiting,
epigastric discomfort, dizziness, headache, skin rash,
thrombocytopenia.
In conditions like Colitis, Crohn’s disease,
Diverticulitis,Gastric ulcer, Diabetes mellitus, Hemorrhoids,
Hepatitis, renal disease, Rectal irritation or bleeding, Systemic
lupus erythematosus,alcoholics and tobacco use: the chance of
adverse effects may be increased.Ibuprofen has been rated as the
safest NSAID by the spontaneous adverse drug reaction reporting
system in UK.
Pregnancy: Contraindicated.
Breast Feeding: May be given.
Man: May be used in reduced dose.
