It is well absorbed orally. It has high protein binding – 90 %.It is extensively metabolized in liver. It is converted in the body to its metabolite desmethyl-imipramine, which is active. About 3 % is excreted in the urine in the form of imino-dibenzylderivatives.
Oral
ALL TYPES OF DEPRESSION, MOOD DISTURBANCE, CHRONIC PAIN, ANXIETY STATE
WITH DEPRESSION, OBSESSIONAL STATES, BULIMAL, ANOREXIA NERVOSA, PHOBIAS:
Intially 25mg t.i.d. increasing slowly to 100mg t.i.d. as reqd.
NOCTURNAL ENURESIS: 7-11 yrs: 35-50mg. Over 11 yrs: 50-75mg, at bedtime,
upto 3 months duration.
Action
Inhibits noradrenaline reuptake and to a lesser extent that of
serotonin. Relatively less sedative than amitryptiline, it reduces REM
sleep and increases stage 4 sleep. Has some anticholinergic action. Can
prolong PR interval and flatten or inverse T wave.
If the drug is administered together with MAOI,
it often produces severe reactions similar to atropine poisoning,
hyperpyrexia, convulsions and coma. Thyroid preparations may
enhance its effects. It can potentiate the action of drugs like
phenylephrine, adrenaline and noradrenaline.

Adverse
Effect & Precautions
Anticholinergic
side effects are common. Dry mouth, tachycardia, palpitation,
impotence, constipation, difficulty in accomomodation, rarely
hyperpyrexia and paralytic ileus. Lethargy, headache, drowsiness,
tremors, ataxia, sweating, convulsion. Urticaria, skin rash,
pruritus, cholestatic jaundice, cardiac arrhythmias, orthostatic
hypotension, agranulocytosis, gynoaecomastia, Loss of weight or
pain.
It should be used carefully in patients with
glaucoma or enlarged prostrate. In pregnancy the drug crosses the
placental barrier and can affect the fetus.
Pregnancy: Use with caution.
Breast Feeding: Use with caution.
Man: May be given in reduced dose.
