After oral administration hepatic first pass metabolism is less than
Isosorbide dinitrate, hence, systemic bioavailability is more.
Oral
PREVENTIVE MANAGEMENT OF ANGINA: Intially 20mg 2-3 times daily or 40mg
b.id, upto 120mg daily in div doses if reqd. Sust. Release tabs: (40/60mg)
1 tab in the morning, increased to 2 tabs if reqd.
Sust. Release tabs: (40/60) i tab in the morning, increased to 2 tabs if
reqd.
Action
Isosorbide 5-mononitrate (IS5MN) is an active metabolite of isosorbide
dinitrate. It relaxes plain muscle by which action it causes dilatation of
the venous capacitance vessels, arterioles and coronary avessels. Preload
and afterload are reduced and coronary flow is redistributed to ischemic
regions. Anginal attacks are reduced and exercise tolerance increased.
Interactions
Alcohol: Severe hypotension and cardiovasuclar collapse.
Aspirin: Increaes serum concentrations of nitrates and their
actions.
Antihypertensives, TCAs, Phenothiazine, Calcium channel blockers:
Marked symptomatic orthostatic hypotension.
Lab Test: May interfere with the Zlatkis-Zakcolour reaction
causeing a false report of decreased serum cholesterol
Adverse
Effect & Precautions
Flushing, throbing headache, palpitation, dizziness, sweating,
skin rash.
Precaution: Withdraw the drug gradually. Start therapy with low
doses.
Pregnancy: Use with caution.
Breast Feeding: Use with caution.
Man: Use with caution.
