Pharmacokinetics



Levodopa is inactive and is converted to active
neurotransmitter(Dopamine) in brain. It undergoes extensive
decarboxylation in periphery after oral administration and only a little
fraction of the dose enters the brain.

Indication
& Dosage

Oral

IDIOPATHIC, POSTENCEPHALITIC AND ARTERIOSCLEROTIC PARKINSON’S DESEASE:
Start with 250mg daily. Increase by 0.5-1g at intervals of 3-4 days till
optimum effective dose is reached. Daily dose should not normally exceed
8g.

Action

Levodopa is a precursor of the striatal
neurotransmitter, dopamine. Unlike the latter leovdopa can cross the blood
brain barrier and gets converted into dopamine in the basal ganglia.
SYmptoms of Parkinson’s disease which are due to dopamine depletion are
relieved.

Interactions



Antacids: Increased bioavailability of levodopa with possible
increase in efficacy.

Anticholinergics: Decreased intestinal absorption of levodopa.

Benzodiazepines: Levodopa’s therapeutic value may be attenuated.

Hydoatoins: Levodopa’s efficacy decreased.

Methionline: Decreased efficacy of levodopa.

Metoclopramide: BIoavailability of levodopa increased while
efficacy of metoclopramide decreased.

MAOIs: Hypertensive reactions occur.

Pyridoxine: Reduced efficacy of levodopa.

TCAs: Hypertensive episodes, decreased bio availability of
levodopa.

Carbidopa: Efficacy of levodopa enhanced.

Bethanidine: Bretylium, guanethedine: Levodopa enhaces hypotensive
effect.

Lab Test: Elevation of uric acid with calorimetric method. Coomb’s
test occasionally positive during extended therapy.

Adverse
Effect & Precautions




Nausea, epigastric distress, vomiting, postural hypotension,
arrhythsion, hightmares & hallucination.



Precaution: Peptic ulcer, Ischemic heart disease. Levodopa therapy
should be stopped 2 days before surgery and anaesthesia (cyclopropane,
halothane) should be avoided in emergency surgery.

Pregnancy: Use with caution.

Breast Feeding: Use with caution.

Man: May be used.
