Pharmacokinetics

 


About 40% of a dose of loperamide
is reported to be absorbed from the gastrointestinal tract to undergo
first-pass metabolism in the liver and excretion in the faeces via the
bile as inactive conjugate; there is slight urinary excretion. Little
intact drug reaches the systemic circulation. The elimination half-life
is reported to be about 10 hours.

& Dosage

Oral

SYMPTOMATIC RELIEF OF ACUTE AND CHRONIC NONSPECIFIC DIARRHOEA ASSOCIATED
WITH INFLAMMATORY BOWEL DISEASE: Initially 4mg followed by 2mg after each
loose motion. Max: 16mg/day. Children under 2 yrs: Not recommended. 2-5
yrs(13-20kg): 1mg t.i.d. 6-8 yrs(20-30kg): 2mg b.i.d. 8-12 yrs (over
30kg): 2mg t.i.d.

Action

Reduces intestinal motility through direct inhibition
of peristaltic activity. Alters water and electrolyte reabsorption from
the colon. Reduces faecal volume.

Interactions

Concomitant
administration with co-trimoxazole increases the bioavailability
of loperamide, (1) apparently by inhibiting its first-pass
metabolism.

Adverse
Effect & Precautions





Skin
rash, intestinal stasis, G.I disturbances.



Precaution: Hepatic dysfunction, acute ulcerative colitis, Crohn’s
disease, intestinalstasis, glaucoma, pyloric obstruction, gastric
retention.

Pregnancy: Safety not established.

Breast Feeding: Safety not established.

Man: May be given in reduced dose.
