Pharmacokinetics

Mefloquine is given as a single oral dose as it has a
long plasma half life of 2 weeks. The absorption of the drug is enhanced
in the presence of food. It is highly bound to the protein and is
concentrated in the RBC. It is metabolised in the liver and is mainly
excreted in the faeces.

Indication
& Dosage








Oral

CHLOROQUINE RESISTANT FALCIPARUM MALARIA, MIXED INFECTION  BY
P.VIVAX AND RESISTANT P.FALCIPAUM
: (250mg tab) Adults (non-immune) upto
60kg: loading dose- 3 tabs & 2 tabs after 6-8 hrs. Over 60kg:
Loading dose-3 tabs, 2 tabs after 6-8 hrs, & i tab after further 6-8
hrs. Adults(semi-immune): Upto 60kg: Single dose of 3 tabs. Over 60kg:
Loading dose of 3 tabs & 1 tab after 6-8 hrs. Children: (under
45kg), irrespective of immunity status, 25mg/kg b.w. (1 tab/10kg b.w.).
Drug should not be taken on empty stomach & dose administered with
atleast 250ml of water.

Action

 


It is a blood schizonticide active against chloroquine-sensitive
and-resistant P.falciparum as well as multi-drug resistant strains. It
is mainly used as a second line treatment in chloroquine-resistant
falciparum infection.

Interactions

Use of
Diltiazem, Flunarizine, Isradipine, Nicardipine,Nifedipine, Nimodipine,
Quinidine, Quinine, Verapamil:
together with mefloquine may result in bradycardia and other cardiac
disorders; also, an increased chance of convulsions may occur when
quinine is taken together with mefloquine. Use of Chloroquine with
mefloquine may increase the chance of convulsions. Use of
Divalproex,Valproic acid together with mefloquine may result in low
blood levels of Valproic acid and an increased chance of convulsions.
Use of oral Typhoid vaccine with mefloquine may reduce the efficacy of
the vaccine.

Adverse
Effect & Precaution




Nausea, abdominal pain,
diarrhoea, dysphonia and dizziness.

Mefloquine may make Convulsions, Epilepsy, Heart
block and Psychiatric disorders worse.

Precaution: Contraindicated.

Breast Feeding: Use with caution.

Man: May be used.
