Pharmacokinetics
Mephentermine acts in about 5 to 15 minutes following intramuscular injection and has a duration of action of up to about 4 hours; it acts almost immediately following intravenous injection with a duration of action of up to about 30 minutes. It is rapidly metabolised in the body by demethylation; hydroxylation may follow. It is excreted as unchanged drug and metabolites in the urine; excretion is more rapid in acidic urine.
Oral
LOW BLOOD PRESSURE: 20mg once or twice daily.
IV Infusion
SHOCK: IN MI, SPINAL ANAESTHESIA, SURGICAL PROCEDURES AND SEVERE ILLNESS: 15-60mg IV drip 5% dextrose
Acts by release of noradrenaline at the adrenergic synapsi. Cardiac output and systolic and diastolic pressures are increased and there is peripheral vasoconstriction. Coronary blood flow is increased and occasionallly there may even be perpheral vasodilation.
Cyclopropane & Halothane: May result in serious ventricular arrhythmias.
Digitalis: Increased reisk of cardiac arrhythmias.
Phenothiaziness, TCAs: May antagonise pressor response of nephentermine.
MAOIs: Potentiate pressor effects of mephentermine.
Oxytotic drugs: May cause severe persistent hypertension.
Shock due to loss of blood or fluid.
Pregnancy: Use with caution.
Breast Feeding: Use with caution.
Man: Use with caution.
