Pharmacokinetics

Pentazocine can be given orally, IM or IV. When
orally given it has an extensive first pass metabolism . Oral to
parental ratio is about 1:4. Pentazocine is conjugated in the liver and
excreted in the urine. Plasma t 1/2 is 3-4 hrs,duration of action after
a single dose is 4-6 hrs.

Indication
& Dosage


 


Oral

MODERATE TO SEVERE PAIN AS IN SURGERY, TRAUMA, COLIC,
BURNS, FRACTURES, CANCER PAIN, SVERE NEURALGIAS: As hydrochloride
25-100mg every 3 or 4 hrs after food. Children 6-12yrs: 25mg 3 or 4 hrly.

IM/SC/IV

INDICATION AS ABOVE: As lactate 30-60mg 3-4 hrly. Children 1yr max
single dose not to exceed 1mg/kg IM or SC AND 0.5mg/kg IV.

Action



A benzomorphan derivative with action similar to
those of morphine though there are important differences. Pentazocine
exerts mixed agoinst-antagonist actions at the opioid receptors. Because
of this pentazocine may cause more dysphoria than euphoria. Withdrawal
symptoms are significantly less severe than with morphine. Pentazocine
has a ‘celling’ effect on respiratory depression i.e., the depression
does not increase with increasing doses.

Interaction

Narcotics will not be effective in people taking
naltrexone. Pentazocine can increase the sedative effect of alcohol and
increase the risk of respiratory depression when used with halothane. It
is incompatible with soluble barbiturate.

Adverse Effect &
Precautions







Dizziness, nausea, vomiting, euphoria, sweating, respiratory,
depre-ssion, sedation, tachycardia, palpitation an hypertension. It has
abuse.


Some of the adverse effects of pentazocine can be
potentially dangerous in patients with CNS disease, head injury,
emphysema, asthma, other chronic lung diseases, enlarged prostate and
difficulty in micturition. The chance of adverse effects may be
increased in patients with Colitis, Heart disease, renal disease and
Hepatic disease.

Pregnancy: Use with caution.

Breast Feeding: Safety not established.

Man: May be given in reduced dose.
