Pharmacokinetics


Primaquine is rapidly absorbed from the GIT. It is concentrated in the
liver, brain, heart, lungs and in skeletal muscle. The drug is
metabolised in the body and is excreted in the urine.Plasma halflife 3-6
hours.It is not a cumulative drug.

Indication
& Dosage






Oral

RADICAL CURE OF VIVAX MALARIA: 7.5mg b.i.d for 14 days. If erythrocytic
forms are present with malarial bouts, a course of chloroquine is used
to control this. Children over 1yr: 0.25mg/kg for 14 days.

TERMINAL PROPHYLAXIS AFTER LEAVING ENDEMIC AREAS: 15mg/day during the
last 2 week of chloroquine prophylaxis. Children under 1yr:

Action



Primaquine is an 8-aminoquinoline which eliminates the
exoerythrocytic forms of malarial parasite P.vivax, by this action
primaquine achieves radical cure of vivax malaria. It is also active
against gametocytes of P.falciparum.

Interaction

Taking Dapsone, Furazolidone, Methyldopa,
Nitrofurantoin,Procainamide, Quinacrine, Quinidine, Quinine,
Sulfonamides, Sulfoxone or vitamin K : with Primaquine may increase the
chance of hemolysis.

Precautions






Nausea, vomiting, weakness, abdominal pain and methaemoglobinaemia.
Haemolytic anaemia in patients with G-6PD deficiency. Passage of dark
urine is indication of haemolysis and primaquine should be stopped.
Leucopenia is seen rarely with larger dose
.


In patients with family or personal history of
hemolytic anemia, Glucose-6-phosphate dehydrogenase (G6PD) deficiency,
Nicotinamide adenine dinucleotide (NADH), methemoglobin reductase
deficiency: There is an increased chance of adverse effects affecting
the blood.

Pregnancy: Safety not established.

Breast Feeding: Contraindicated.
