Pharmacokinetics

 



After oral administration is distributed to different tissues and
penetrates meninges, caseous masses and placental barrier. It is
metabolised in liver to active metabolite and induces the microsomal
enzymes of liver.

Indication
& Dosage


 



Oral

TUBERCULOSIS: Part of multidrug regimens: Adult450 or 600mg/day for
the first 2 month and then 10-15mg/kg three times a week for tfurther
4-6 months. Max: 900mg/day. Children 10-20mg/kg daily. Max: 600mg/day.

LEPROSY: Given along with clofazmine and dapsone. Adults over 35kg:
450mg once a month.Children 10-14yrs: 450mg once a month. Low weight:
12-15mg/kg once a month.

PROPHYLAXIS OF MENINGOCOCCAL INFECTIONS: Adults: 600mg b.i.d for 2 days.
Children 1-12 yrs: 10mg/kg b.i.d infants 3-12 months: 5mg/kg b.i.d
PROPHYLAXIS OF MENINGITIS DUE TO H.INFLUENZAE TYPE B: 20MG/KG once a day
for 4 days. Neonates under 1 mont: 10mg/kg/day

Action

 


Bactericidal action against a wide range of organism. Can kill
intracellular organisms as also semidormant or persistent ones.
Generally reserved for the treatment of tuberculosis and leprosy and
oppotunistic atypical mycobacterial infections such as those in HIV
POSTIVE OR AIDS patients. Inhibits the enzyme DNA-dependent RNA
polymerase reusulting in suppression of nucleic acid synthesis.

Interaction

 

Rfampicin by inducing the hepatic microsomal enzymes may reduce the
efficacy of the following drugs: Acetaminophen, oral anticoagulants,
barbiturates, penzodiaze-pines, chloramphenicol, cofibrate. Oral
conraceptives, corticosteroids, cyclosporine, digitoxin, disopyramide,
beta blockers oestrogens, hydantoins, mexiletine, quinidine, sulfones,
sulphonylureas, theophylline, verapamil.

Halothane: Hepatotoxicity and hepatic encephalopathy.

Isoniazid: Higher incidence of hepatotoxicity than wiht either agent
alone.

Ketoconazole: Treatment failure of either ketoconazole or refampicin.

Food: Interferes with absorption of rifampicin, therefore it should be
taken on empty stomach.

Lab Test: Inhibits standard assays for serum folate and vitamin B12.
Transient abnornalities in liver fuction tests, reduced excretion of
contrast media used for visualization of gall bladder

Adverse
Effect & Precautions




Skin rash, drug fever, nausea, vomiting, peripheral neuropathy, fatigue
, hepatitis and jaundice. Haemolytic anaemia. Diarrhoea, drowsiness,
ataxia, headache, flu like syndrome, stomatitis.



Precaution: Liver impairmetn, imparts orange  red colour to urine,
saliva, tears sweat and sputum. Malnourished. Young children. Contact
lenses may be irreversibly stained.

Pregnancy: Use with caution.

Breast Feeding: Use with caution.

Man: Use with caution.
