IV
ACUTE BLEEDING OESOPHAGEAL VARICES: IV bolus of 250mcg over 1 min.,
followed by 6mg/24hr continous infusion x 4 days. Infusion should be
given. UPPER GI HEMORRHAGE FROM GASTRIC/DUODENAL ULCERS: IV bolus of
250mcg (3.5mcg/kg) followed by 3 mg 12 hourly continuous infusion x 2.5d
days; as necessary INTESTINAL & PANCREATIC FISTULAE: IV bolus not
reqd. Infusion(6mg/d) may be reqd. upt o 14 days. After closure of
fistulae infusion should be at half-dosage for the next 48 hrs.
Action
It is not only a hpothalamic hormone but also present in
g.i.t. & pancreas. It inhibits the release of GH, & its has also
been effective in bleeding oesophageal varices & GI haemorrhages.
Its beneficial effect in variceal hemorrhage is due to constriction of
splanchnic artery, which in turn reduces its blood flow & hepatic
blood flow, with subsequent lowering of variceal pressure.
Adverse
Effect & Precautions
Fluid retention, hyponatraemia on
administration, hyponatraemia on administration without restricting
fluid intake, stomach ache, headache, nausea. vomiting, epistaxis.
Alergic reactions ranging from urticaria to anaphylaxis may occur.
Precaution: Renal impairment, cardiovascular disease and
hypertension(not indicated for nocturnal enuresis and nocturia in those
over 65 years), cystic fibrosis.
Pregnancy: Contraindicated.
Breast Feeding: Contraindicated.
Man: Contraindicated.
