Pharmacokinetics.

After oral
administration the bioavailability is poor due to first pass effect.
About 80% sumatriptan is eliminated as metabolites and 20% of the drug
is excreted unchanged.

Indication
& Dosage




Oral

ACUTE TREATMENT OF MIGRAINE ATTACK:
Start with 25-50mg. A second
dose of upto 100mg may be given if necessary, after 2 hours. Additional
doses may be given at 2 hourly intervals upto max daily dose of 300mg

SC

ACUTE TREATMENT OF MIGRAINE ATTACK: 6mg SC at
the onset of attack. A second dose may be taken after a gap of at least 1
hour. Maximum dose in one day is 12mg.

Action



Sumatripatan is a selective agonist for a subtype of serotonin
(5-HT) receptor(probably 5-HT1 D) in cranial blood vessels. It causes
vasoconstriction and reduced blood flow in both extracranial and
intracranial tissues. The antimigraine action could also involve
vasoconstriction of disteded dural vessels. Sumatriptan also blocks plasma
protein extravasation from dural vessels. Sumatriptan increases systolic
and diastolic blood pressures. A small but significant delay in gastric
emptying also occurs.

Interaction

Ergotamine
containing preparations: Concomitant use should be avoided because of
vasopastic reactions.

MAOIs: Concomitant use within 2 weeks of discontinuationof MAOIs is
contraindicated.

Lithium: Concomitant use must be avoided.

Adverse
Effect & Precautions


 

With S.C. inj: The commonest is
inj. site reaction. Thngling, warm or hot sensation, dizziness, fatigue
& manlaise.

With oral: Nausea, vomiting, taste disturbances, fatigue malaise,
dizziness, drowsiness, chest tightness and pressure. It may cause coronary
vasopasm inpatients with history of coronary artery disease. There have
been reports of serious and/or life theratening arrhythmias.



Precaution: Hepatic or renal impairment, to use with caution inpatients
with history of sensitivity to multiple allergens. There are no well
controlled sutdies of sumatriptan in children

Pregnancy: Safety not established.

Breast Feeding: Safety not established.

Man: May be given in reduced dose.
