Tinidazole




 










Pharmacokinetics

Tinidazole has a longer half life than Metronidazole
and is better tolerated. Its t1/2 is greater than 12 hrs, hence used for
single dose therapy. It is metabolised in the liver through
glucuronidation and Oxidation and is excreted in the urine.

Indication
& Dosage




Oral

AMEBIASIS: 2g once daily for 3-5 days.


TRICHOMONIASIS:
2g as a single dose.


GIARDIASIS:
Adult: 2g single dose.


Children:
50-75mg/kg single dose.


ANAEROBIC INFECTIONS:
2g initially follwed by 1g/day for 5-7 days.


BACTERIAL PROPHYLAXIS:
2g single dose.


SURGICAL PROPHYLAXIS:
2g given 12 hrs before surgery.




IV

SURGICAL PROPHYLAXIS:
1.6g given as infusion before surgery.


ANAEROBIC BACTERIAL INFECTIONS WHEN ORAL THERAPY IS NOT POSSIBLE
(PERITONITIS, ABDOMINAL ABSCESS, BRAIN ABSCESS ETC) :
800mg infused as
400ml solution (2mg/ml) at 10ml/min. This is followed by 800mg daily until
oral therapy can be given.

Action

Highly active against Entamoeba histolytica. Trichomonas vaginalis,
Giardia lambila and anaerobic bacteria such as gardnerella vaginalis,
Clostridium species. Tinidazole is converted to the active metabolite by
reduction at the 5-nitro position. The active metabolite causes DNA damage
in pathogens.

Interactions

 

Tinidazole increases the risk of bleeding when
combined with anticoagulants. When the drug is taken along with
alcohol, disufiram like reaction occurs. Enzyme inducers (
phenobarbitone, rifampicin ) and cimetidine can reduce the
metabolism of tinidazole.

Adverse
Effect & Precautions




Nausea, epigastric
discomfort, metallic taste, furred tongue, neuropathy, skin rash,
uriticaria, angioedema, leucopenia, seizures on prolonged
dyscrasias.


 

Precaution has to be taken in late trimesters
of pregnancy, seizure disorders, in hepatic and renal impairment
and in alcoholics.

Pregnancy: Contraindicated in 1st trimester of pregnancy.

Breast Feeding: Use with caution

Man: May be used in reduced dose.
