Tramadol is readily absorbed following oral administration but is subject to first-pass metabolism. Tramadol is metabolised by N- and O-demethylation and glucuronidation or sulphation in the liver. The metabolite O-desmethyltramadol is pharmacologically active. Tramadol is excreted mainly in the urine predominantly as metabolites. Tramadol is widely distributed, crosses the placenta, and appears in small amounts in breast milk. The elimination half-life following oral administration is about 6 hours.

& Dosage
Oral
MODERATE TO SEVERE PAIN & POST-OP. PAIN: 50-100mg 3-4 times daily.
SC/IM/IV
SEVERE ACUTE OR CHRONIC PAIN, CANCER PAIN, POST OPERATIVE PAIN: Adult and
adolscents over 14 yrs: Single dose 100mg given sc, im or slow IV bolus or
infusion. Maximum daily dose. 400mg. Children: 1-1.5mg/kg.
NOTE: Renal or hepatic impairment may require dose reduction.
Tramadol acts a weak agonist at all types of opioid receptors with some selectivity for the nu-receptors. It has excellent analgesic activity but little repiratory depression or gatrointestinal effects. Tolerance to the analgesic effect does not develop during prolonged therapy and there is no dependence.
Interactions
Carbamazepine: A
significant increase in Tramadol metabolism.
Hypnotics, Tranquilizers and other CNS depressants: Sedative
effects potentiated.
MAOIs: Fatal interaction occurs. Opioid analgesics like
Buprenorphine, Butorphanol and Pentazocine: May precipitate
withdrawal symptoms.
Tranquilzers: Analgesia potentiated.
Adverse
Effect & Precautions
Nausea, vomiting
and dizziness.
Precaution: Alcohol. Not to drive and operate machinery.
Pregnancy: Safety not established.
Breast Feeding: Safety not established.
Man: May be given in reduced dose.
