Pharmacokinetics
Amitriptyline, after oral
administration, is distributed widely and is metabolised ot
nortriptyline whic is active.
DEPRESSION: Initially 25mg 3 times daily. This may be increased upto 150mg daily in divided doses. Dose increments should be made in the late afternoon and/or at bedtime. Elderly and adolescents: 10mg t.i.d may be sufficent. Children: Below 12yrs amitriptyline is not recommended.
Amitriptyline exerts its antidepressant action by blocking the neuronal reuptake of noradrenaline and sertonin. Amitriptyline has some sedative action. Endogenous depression responds to a greater extent than other types. Amitriptyline has significant anticholinergic activity.
Adverse
Effect & Precaution
Epigastrid distress,
sedation, drowsiness, orthostatic hypotension, palpitation, dry mouth,
urinary retention, blurred vision, confusion, lowering of seizure
threshold.
Precaution: Cardiac, hepatic or renal
disease. Not to drive or operate heavy machinery, hpethyroidism,
activation of latent schizophrenia may occur, suicidal tendency in
seriously depressed patients.
Pregnancy: Safety not established
Breast Feeding: Use with caution
Man: May be given in reduced dose
