Pharmacokinetics

 


Ampicillin is water
soluble & acid resistant.It is partially absorbed from the GIT. Its
absorption is not interfered with by food. It gets excreted unchanged in
the urine.

Indication and Dosage




Oral

GENERAL DOSAGE:
Adults 250-500mg 3-4 times/day. Higher doses upto
1g 6 hourly may be given.

UPPER AND LOWER RTI-BRONCHITIS, LUNG ABSCESS,
PNEUMONIA ETC:
General dosage.

OTITIS MEDIA: Children: 125mg t.i.d to 750mg
b.id. for 3-10 days

SINUSITIS: General dosage. TONSILLITIS,
PHARYNGITIS: General dosage. In the latter condition duration of treatment
should be atleast 10 days.

UTI: General dosage. UNCOMPLICATED GONORRHOEA:
Single dose of 3g.

SKIN AND SOFT TISSUE INFECTIONS: General dosage, because of drug
resistance not the first choice Response may be slow.



IV

BACTERIAL MENINGITS:
2g IV every 6hrs in adults and half of this in
children. Not the first choice.

Action

 


Ampicillin exerts bactericidal action on both gram-positive and
Gram-negative organisms. Its spectrum includes Gram-positive aerobic and
anerobic organisms such as staphylococci, strep Faecalis, Peptococcus,
Peptostreptococcus, C. diphtheriae, B.arthracis, Listeria, Clostridium
tetani, CI.perfringens, CI.botulinum. Ampicillin is active against the
following Gram-negative organisms: Moraxella catarrhailis, Neisseriae,
Pasteurella multocida, some strains of E coli, nonbetalactamse-producing
proteus mirabilis, Salmonella spp., S.typhi, Shigella spp., Brucella spp.,
B. pertussis Vibrio cholerae, H. influenzae type b, Gardnerella vaginalis,
etc. Ampicillin is not active against Enterobacter, Klebsiella, Serratia,
Citrobacter, most spp. of Proteus, Yersinia and Pseudomonas. Ampicillin
exerts its action by inhibiting the synthesis of bacterial cell wall.

Interaction

 

Oral contraceptives containing Estrogens : Use of ampicillin together with
estrogen-containing oral contraceptives may prevent oral contraceptives
from being effective, increasing the chance of pregnancy. Methotrexate :
Use of Methotrexate with Ampicillin may increase the chance of adverse
effects of Methotrexate. Probenecid : Probenecid causes Ampicilin to build
up in the blood. This may increase the risk of adverse effects.

Adverse
Effect &Precaution
 



Nausea, epigastric distress,
diarrhoea, skin rash, durg fever, urticaria, oral candidiasis.


 


In Allergy: Patients with a history of general allergies may be more
likely to have a severe reaction to Ampicillin. Renal disease: Patients
with renal disease may have an increased chance of adverse effects.
Mononucleosis : Patients with infectious mononucleosis may have an
increased chance of developing a skin rash when receiving ampicillin.
Stomach or intestinal disease, history of (especially colitis, including
colitis caused by antibiotics) :Patients with a history of stomach or
intestinal disease may be more likely to develop colitis while taking
Ampicillin.

Pregnancy: May be used.

Breast Feeding: May be used.

Man: May be used
