Pharmacokinetics

 

Astemizole
when given orally has a slow onset of action (2-4hrs) and a very long
duration of action of about 2-5 days. It is highly plasma protein bound
( 97%). It has plasma halflife of 20 hrs. It has an active metabolite
which has a half life of 12-19 days. The drug is mainly excreted through
faeces.

Indication and Dosage


 


Oral

ALLERGIC RHINITIS SEASONAL AND PERENNIAL, ALLERGIC
CONJUNCTIVITIS, CHRONIC URITCARIA AND OTHER CHRONIC ALLERGIES:
10mg
once daily.

Children: 6-12 years. 5mg once daily.

Children 2-6 years: 1mg/5kg/day.

Action

Astemizole is a
long-acting, selective histamine H1-receptor antagonist. It is a second
generation antihistamine in that it does not readily cross the blood-brain
barrier. Astemizole causes less sedation and psychomotor impairment. Onset
of action is slow which makes it sutable for use aon a regular basis than
on demand rather

Interactions

 

Use of
Quinine, Azithromycin, Clarithromycin, Erythromycin, Itraconazole
and Ketoconazole with astemizole may cause cardiac problems, such
as arrhythmias ; hence these drugs should not be used together.

Adverse
Effect & Precautions




Weight gain, increased
appetite, faltulence, serious ventricular arrhythmias at very high
dose.




 
 



Precaution has
to be taken in urinary tract problems, prostatic hypertrophy and
in Glaucoma. In hepatic disease the level of astemizole is
increased thus increasing its toxicity. Use of astemizole in those
with hypokalemia may cause arrhythmias.

Pregnancy: Contraindicated.

Breast Feeding: Contraindicated.

Man: May be give in reduced dose
