Pharmacokinetics


 

Cefaperozone is given predominantly Intravenously. It penetrates tissues
well. The half life is 2 hrs. 90% is protein bound. 25 % is excreted in
urine.Its excretion is reduced in renal impairment. It is metabolised in
the liver.

Indication
& Dosage


 



IM/IV

UPPER AND LOWER RESPIRATORY TRACT INFECTIONS, UTI, INTRA-ABDOMINAL
INFECTIONS INCLUDING PERITONITIS, CHOLECYSTITIS, CHLOANGITIS,
SEPTICAEMIA, MENINGITISM SKIN AND SOFT TISSUE AND BONE AND JOINT
INFECTIONS PELVIC INFLAMMATORY DISEASE, GONORRHEA, SURGICAL PROPHYLAXIS
AND INFECTIONS IN IMMUNOCOMPROMISED PATIENTS:
Adults: 2-4g in 2 divided
doses. In severe infections upto 6-12g or even 16mg/day may be given in
2-4 divided doses. Children & Neonates: 50-200mg/kg/day in 2 equally
divided doses 12 hours apart; upto 300mg/kg/day has been used in severe
infections including bacterial meningitis.

Renal impairment: No change in dosage required.

GONORRHOEA: 500mg IM single dose. SURGICAL PROPHYLAXIS: 1 or 2g IV 
30-40 min before surgery. May be repeated every 12 hours for upto 72
hours. IV solutions hsould have concentrations from 2-25mg/ml of
cefoperazone for continouous infusion. If cefoperazone for continuous
infusion. if cefoperazone has to be given IM at concentrations of more
than 250mg/ml, lignocaine may to added.

Action

 


Has a broad spectum of activity against most Gram-positive,
Gram-negative and anaerobic organisms. It is suitable for empirical
treatment of infections by unidentified pathogens. Active against
betalactamase-producing pathogens also. High biliary concentrations.
Active against pseudomonas aeruginosa.

Interaction

 

It enhances the toxicity of Aminoglycosides. It causes Disulfiram like
reaction with alcohol.

Adverse
Effect & Precautions




Skin rash, urticaria,
transient rise in liver enzymes & reversible neutropenia.


 

Precaution to be taken in infants less than 1 month.It causes
hypoprothombinemia and increases the bleeding tendency.

Pregnancy: Use with caution.

Breast Feeding: Use with caution.

Man: May be used.
