Indication
& Dosage


 


Oral; IV

MENINGITIS DUE TO H. INFLUENZAE, OTHER INFECTION CAUSED BY SUSCEPTIBLE
ORGAINISMS, TYPHOID FEVER: Adult: 50mg/kg/day in 4 divided doses
given 6 hourly. Children (over 1 month): 75mg/kg/day in 4 equal doses
given 6 hourly. Neonates(under 2 weeks): 25mg/kg/day IV every 24 hours.
2-4 weeks: 50mg/kg/day IV in or oral in four divided doses. Duration:
Typhoid 8-10 day, meningitis 7-10 days, brain absces abscess upto 4
weeks.

Action

 


Bactericidal or bateriostatic by inhibition of protein synthesis.
Susceptible organisms include Neisseriae, Strep-pneumoniae, H.Influenzae,
Staphylococci, Streptococci, Salmonella, Shigella, Anaerobes,
Ricketssiae and Bruecella. Not effective against pseudomonas.

Interactions

 


Chloramphenicol is inactivated in the liver and may, therefore, interact
with drugs that are metabolised by hepatic microsomal enzymes. For
example, chloramphenicol enhances the effects of coumarin anticoagulants
such as dicoumarol and warfarin sodium, some hypoglycaemics such as
chlorpropamide and tolbutamide, and antiepileptics such as phenytoin,
and may reduce the metabolism of cyclophosphamide to its active form.
Conversely the metabolism of chloramphenicol may be increased by
inducers of hepatic enzymes such as phenobarbitone or rifampicin.
Contradictory results have been reported with paracetamol and phenytoin.

Chloramphenicol may decrease the effects of iron and vitamin B(12) in
anaemic patients and impair the action of oral contraceptives.

Adverse
Effect & Precaution


 




Nausea, vomiting, epigastric distress, skin rash, gray baby syndrome,
superinfection, aplastic anaemia, pancytopenia, optic and peripheral
neuritis.



Precaution: Repetitive course to be avoided, duration of therapy less
than 2 to 3 weeks. Periodic blood counts. Not to be used in minor
infections, and concmitantly with phenytoin, anticoagulants and
sulfonylureas

Pregnancy: Contraindicated.

Breast Feedign: Contraindicated.

Man: May be used
