Pharmacokinetics

 


Chlorthalidone is erratically absorbed from the gastrointestinal tract.
It has a prolonged elimination half-life from plasma and blood of 40 to
60 hours and is highly bound to red blood cells; the receptor to which
it is bound has been identified as carbonic anhydrase. Chlorthalidone is
much less strongly bound to plasma proteins. Chlorthalidone is mainly
excreted unchanged in the urine. It crosses the placental barrier and is
distributed into breast milk.

Indication
& Dosage


 


Hypertension, oedema of hepatic, renal and cardiac orgin, ascite due to
cirrhosis.

Dosage: Adults: Oedema: Intially 50mg in the morning or 100-200mg on
alternate days, reduced for maintenance.

Hypertension: 25mg, increased to 50mg. If necessary, in the morning.

Children: 2mg/kg/day given three times a week.

Interaction

 


Cholestyramine & Colestipol: decrease absorption of chlorthalidone.

Diazoxide: Additive action may cause hyperglcemia, hyperuricaemia and
hypotension.

Digitalis: Diuretic induced hypolalaemia may precipitate digtalis
toxicity.

Lithium: Chlorthalidone toxic effects by decreasing its renal excretion.

Frusemide: Synergy leading to profound diuresis and greater than
predicted electrolyte loss.

Non-depolarizing muscle relaxants: Diuretic induced hypolalaemia
enhances efficacy.

Adverse
Effect & Precaution


 


Nausea, vomiting, epigastric, distress, weakness, fatigue, dizziness,
impotence, jaundice, skin rash, electrolyte imbalance and visual
disturbance.

Precaution: Renal and hepatic disease, gout and diabetes.

Pregnancy: Contraindicated in hypetension pregnancy.

Breast Feeding: Use with caution.

Man:Use with caution.
