Pharmacokinetics

 


It is well absorbed orally (60-80 %), but its bioavailablity is reduced
by about 1/3 when given with antacids. Theraupetic effect lasts for
about 5-8 hours. It is excreted mainly in the urine, within 24
hours,mainly as unchanged drug. It can cross the placental barrier and
is secreted in milk.

Indication
& Dosage

Oral

DUODENAL AND GASTRIC ULCERS: 800mg daily at bedtime for 4-8 weeks.
This may be followed by 400mg daily at dedtime.

REFLUX OESOPHAGITIS: 400mg q.i.d. with meals and at bedtime for 4-8
weeks. 

ZOLLUNGER-ELLISON SYNDROME:  400mg q.i.d may be increased upt
o2.4g/day.

STRESS ULCER PREVENTION: 200-400MG ORALLY or nasogastric tube fevry 4-6
hrs

PREVENTION OF ACID ASPIRATION DURING GENERAL INTRACTABLE ULCERSIN
HOSPITALISED PATINETS. 200MG 4-6 hrly. Max 2.4g/day.



IM

PATHOLOGICAL HYPERSECRETORY CONDITIONS AND INTRACTABLE ULCERS IN
HOSPITALISED PATIENTS. 200mg 4-6 hrly. Max 2.4g/day.

IV

PATHOLOGICAL HYPERSECRETORY CONDITIONS AND INTRACTBLE ULCERS IN
HOSPITALISED PATIENTS. 200mg slow IV inj, given over 2 mins, may be
repeated 4-6 hrly. Max: 2.4g/day. Infusion: 400mg in 100ml Sod.Chl 0.9%
infused over half to one hour. May be repeated 4-6 hrly. Continous
infusion at an average rate of 5-100mg/hr over 24 hrs. Max 2.4gms.
Children under 1 yr: (IM Or Slow yr: 25-30mg/kg/day in div doses.

NOTE: Renal impairment. Dose reduction.

Inhibits gastric acid secretionand reduces pepsin output.

Interaction

 


Aminophylline, Anticoagulants, Caffeine, Metoprolol, Oxtriphylline,
Phenytoin, Propranolol, Theophylline, Tricyclic antidepressants (amitriptyline,
amoxapine, clomipramine, desipramine, doxepin, imipramine, nortriptyline,
protriptyline ) : these drugs when used with Cimetidine has been shown
to greatly potentiate the effects of Cimetidine.H2-blockers may decrease
the effects of itraconazole and ketoconazole, hence H2-blockers should
be taken at least 2 hours after these drugs.

Adverse Effect
& Precaution




Skin rash, muscular pain and
nausea, diarrhoea, confusion, disorientation, delusion, hallucination,
convulsion, impotence, gynaecomastia.

  

Allergies to Cimetidine can occur. Precaution during Breast-feeding :
Cimeitidine passes into the breast milk and may cause adverse effects in
the nursing baby. In Adults : Confusion and dizziness may be occur.

Pregnancy: Safety not established.

Breast Feeding: Use with caution.

Man: May be used in low dose
