Dipyridamole



 












 

Pharmacokinetics

 

Dipyridamole is
incompletely absorbed from the gastrointestinal tract with peak plasma
concentrations occurring about 75 minutes after oral administration.
Dipyridamole is highly bound to plasma proteins. A terminal half-life of
10 to 12 hours has been reported. Dipyridamole is metabolised in the
liver and is mainly excreted as glucuronides in the bile. Excretion may
be delayed by enterohepatic recirculation. A small amount is excreted in
the urine.

Indication
& Dosage


 



Oral

LONG TERM PREVENTION OF CORONARY INSUFFIECIENCY, POST MI FOR CORONARY
DILATATION AND TO MIMPROVE COLLATERAL CIRCULATION: 50mg t.i.d before
meals. 

INHIBITION OF PLATELET AGGREGATION:  375-400mg/day in divided
doses.

Action

 


Dipyridamole
inhibits platelet aggregation. It also reduces peripheral and coronary
vascular resistance. Coronary flow is increased but myocardial perfusion
distal to occluded arteries may be worsened. A ‘steal’ phenomenon may
occur.

Interactions

 


Oral
anti-coagulants & anti-arrhythmic agent Adenosine : Efficacy
enhanced by dipyridamole. 

Antacids : Reduce efficacy of dipyridamole.

Aminophylline : Reverses vasoilatation effect of dipyridamole.

Aspirin : Useful combination for prevention of thromboembolic
phenomenon.

Precaution

 


Nausea,
dizziness, skin rash headache.

  


Subvalvular aortic
stenosis, rapidly progressing angina, recent MI associated with
haemodynamic instability.

Pregnancy : Safety not established.

Brest Feeding : Use with caution.

Men : May be used.
