 Pharmacokinetics

Given orally has high hepatic metabolism. Hence
higher dose by oral route should be used than by I.V route. It is highly
protein bound.

& Dosage




Oral

PREVENTION OF NEUROLOGICAL DEFICITS AFTER SUB ARACHNOIDHAEMORRHAGE, ACUTE
ISCHEMIC STROKES, SENILE DEMENTIA:
60mg 4 hrly for 3-4 weeks starting
within 4 days of subarachnoid hemorrhage. Other conditions: 30mg 3-4 times
daily.

TREATMENT OF ISCHAEMIC NEUROLOGICAL DEFICITS FOLLOWING SUB-ARACHNOID
HAEMORRHAGE: Infusion via central catheter 1mg/hour initially, increased
after 2hrs to 2mg/hr, constantly monitoring BP. Start treatment early
and  continue for min 5 days(max 14 days). Patients under 70kg or
with unstable BP: Initiate with 0.5mg/hr infusion and adjust as reqd.

Action

Inhibits inflow of ca2+ into cell by blocking calcium
channels resulting in relaxation of vascular smooth muscle. Begin highly
lipophilic nimodipine has greater action on the cerebral vessels.

Interactions

Interaction with the following drugs have been
reported: Acetazolamide, Amphotericin B, Corticosteroids,
Dichlorphenamide, Diuretics, Methazolamide, Beta-blockers like
acebutolol, atenolol, betaxolol, carteolol, labetalol, metoprolol,
nadolol, oxprenolol, penbutolol, pindolol, propranolol, sotalol,
timolol : Effects of both may be increased. In addition, adverse
effects may occur if a calcium channel blocker or a beta-blocker
is stopped suddenly after use together. Carbamazepine,
Cyclosporine, Procainamide,Quinidine: Effects of these drugss may
be increased if they are used together with calcium channel
blockers. Digitalis glycosides : Effects of these drugs may be
potentiated if they are used with calcium channel blockers.
Disopyramide: Effects of calcium channel blockers on the heart may
be potentiated.

Precautions

Allergies : Allergic reaction to nimodipine may
occur. Pregnancy: Calcium channel blockers have not been studied
in pregnant women. However, studies in animals have shown that
large doses of calcium channel blockers cause birth defects,
prolonged pregnancy, poor bone development, and stillbirth.
Breast-feeding : Nimodipine passes into breast milk,but they have
not been reported to cause problems in nursing babies. Older
adults: Elderly people may be especially sensitive to the effects
of calcium channel blockers. This may increase the chance of
adverse effects during treatment.
