Treatment of Detrusor Sphincter Dyssynergia by Transperineal Injection of Botulinum Toxin

Introduction – Detrussor sphincter dyssynergia [DSD] is an involuntary contraction of the external urethral sphincter that occurs during the detrusor contraction, causes voiding dysfunction that may result in urological complications like hydroureteronephrosis and renal failure.
Results – Transperineal injections of botulinum produced improved bladder function in all patients. After treatment urodynamic evaluation showed an increase in functional detrusor capacity and a reduction in the maximal detrusor pressure during voiding.
Conclusions – Botulinum toxin must be considered in the treatment of DSD. As it is reversible it must be considered before stent use or surgery.
