Premature Rupture of the Membrane

   

  • An increase in fetal plasma cortisol but not dehydroepiandrosterone sulfate is followed by the onset of preterm labour in patients with preterm premature rupture of the membranes.

    Fetal plasma levels of cortisol and fetal plasma cortisol/DHEA-S ratio are dramatically increased in women with preterm premature rupture of the membranes. These increases are followed by the spontaneous onset of preterm labour. The increased fetal plasma cortisol/DHEA-sulphate ratio indicates that the fetus whose mother has preterm PROM has an endocrine response similar to that of a critically ill adult.

        


    Editor comments : The study confirms that pregnancies with preterm premature rupture of the membranes have a large risk of intrauterine infection – 19.6% here and that the fetal response includes pituitary adrenal activation. The study shows nothing about onset of human labour in the absence of preterm PROM or infections.
